Line 6: ID Number. The disclosure of the last four digits of your social security number is required. [KSA 25-2309 (b)(5)]. It is used for administrative purposes only and is not released to the public [KSA 25-2309 (j)].

Line 7: Choice of Party. You must register with a party if you want to take part in that party’s primary election, caucus, or convention.

Line 8: Race or Ethnic Group. Leave blank.

Line 9: Signature. To register in Kansas you must:

1) be a citizen of the United States; 2) be a resident of Kansas; 3) be 18 by the next statewide general election; 4) have completed the terms of your sentence if convicted of a felony; a person serving a sentence for a felony conviction is ineligible to vote; 5) not claim the right to vote in any other location or under any other name; 6) not be excluded from voting for mental incompetence by a court of competent jurisdiction.
