VOTER REGISTRATION REQUIREMENTS

Registration Deadline. 20 days before the election.
Line 6: ID Number. The last four digits of your social security number and your Utah driver’s license or identification card number are requested. The disclosure of these numbers is voluntary and requested by authority of Utah Code Annotated 20A-2-104. These numbers are used to maintain the accuracy of the registration records.
Line 7: Choice of Party. Declaring a party is not required in order to register to vote. However, Utah's election law allows each political party to choose whom it will allow to vote in its primary election. If you do not affiliate with a party, you may be restricted from voting in the primary.
Line 8: Race or Ethnic Group. Leave blank.
Line 9: Signature. To register in Utah you must:
1) be a citizen of the United States; 2) have resided in Utah for 30 days immediately before the next election; 3) be at least 18 years old on or before the next election; 4) not be a convicted felon currently incarcerated for commission of a felony; 5) not be convicted of treason or crime against the elective franchise, unless restored to civil rights; 6) not be found to be mentally incompetent by a court of law.
