VOTER REGISTRATION REQUIREMENTS

Registration Deadline. For municipalities where voter registration is required, 13 days before the election (or completed in the local voter registration office up to 5:00 pm. 1 day before the election, or completed at the polling place on election day).

Line 6: ID Number. Leave blank.

Line 7: Choice of Party. Not required.

Line 8: Race or Ethnic Group. Not required.

Line 9: Signature. To register in Wisconsin you must:

1) be a citizen of the United States; 2) be a resident of Wisconsin for at least 10 days; 3) be 18 years old; 4) not have been convicted of treason, felony or bribery, or if you have, your civil rights have been restored; 5) not have been found by a court to be incapable of understanding the objective of the electoral process; 6) not make or benefit from a bet or wage depending on the result of an election
